Job summary

GoldLeaf Homecare Network is seeking a Client Care Manager to be the heart of the care experience in Mountain View. You’ll lead client care operations, guide care plans, and match caregivers to families to ensure safe, dignified in-home care across the Peninsula and South Bay.

You’ll supervise caregivers, recruit and onboard new staff, and maintain strong relationships with families and referral partners. This hybrid role combines office work with client home visits and regular on-call duties.

Qualifications

• 4+ years of care management or client services experience.
• Valid driver’s license and reliable transportation.
• Ability to pass a background check.
• Proficient English communication skills.

Responsibilities

• Conduct intake calls and in-home assessments to understand client needs.
• Create and maintain personalized care plans reflecting goals and changes.
• Oversee caregiver scheduling and client matching; ensure coverage.
• Lead after-hours on-call rotation and respond to urgent client needs.
• Coach and develop caregivers; conduct performance reviews.
