Care Options for Kids is seeking a Client Success Associate to manage end-to-end referrals and admissions for private duty nursing services in the home. You will act as a liaison among referral sources, patients, families, clinical teams, and payers.

This remote role supports PA/NJ/DE and requires coordinating intake from referral to service activation, verifying eligibility and benefits, and tracking authorizations. Strong EMR and MS Office skills are essential.

Formación

• Associate degree required; bachelor’s degree preferred.
• Minimum of two years of experience in healthcare intake, admissions, care coordination, home health, PDN, or managed care preferred.
• Knowledge of insurance verification, authorization processes, Medicaid programs, managed care organizations, and PDN related health plans preferred.
• Strong organizational, communication, and interpersonal skills.
• Proficiency with EMR systems and Microsoft Office applications (Outlook, Excel).
• Ability to travel occasionally (up to 10%).

Responsabilidades

• Receive and manage incoming referrals from hospitals, physicians, case managers, managed care organizations, and community partners.
• Coordinate and manage all new patient intakes from referral through admission and service activation.
• Review referral documentation for completeness and accuracy, and enter all required information into the EMR.
• Verify patient eligibility, benefits, and insurance coverage, including Medicaid and commercial plans.
• Obtain and track authorization requests and Single Case Agreements from initiation through execution.
• Serve as primary contact for patients, families, and referral sources regarding services and admission status.
• Maintain ongoing communication with referral sources, families, and internal teams regarding intake status and readiness.
• Ensure compliance with agency policies, regulatory requirements, and payer guidelines.
• Complete Intake Workflow tasks in the EMR and identify process improvements.
• Track and maintain intake records, metrics, and reporting data.
