Job summary

Care Options for Kids is seeking a Client Success Associate to manage the end-to-end referral and admission process for private duty nursing in-home services. You will coordinate intake from referral to activation and serve as the liaison among referral sources, families, clinical teams, and payers.

You will verify eligibility and benefits, handle authorizations, and maintain compliant intake records while driving process improvements and clear communications with stakeholders.

Qualifications

• Associate degree required; bachelor’s degree preferred.
• Minimum two years of healthcare intake, admissions, care coordination, home health, PDN, or managed care experience.
• Knowledge of insurance verification, authorization processes, Medicaid, PDN-related plans.
• Strong organizational, communication, and interpersonal skills.
• Proficiency with EMR and Microsoft Office (Outlook/Excel).
• Ability to communicate effectively in English; bilingual a plus but not required.

Responsibilities

• Receive and manage incoming referrals from hospitals, physicians, case managers, managed care, and community partners.
• Coordinate and manage all new patient intakes from referral through admission and activation of services.
• Review referral documentation for completeness and accuracy and enter data into the EMR.
• Verify patient eligibility, benefits, and insurance coverage, including Medicaid and commercial plans.
• Obtain, submit, and track authorization requests and Single Case Agreements.
• Serve as primary contact for patients, families, and referral sources regarding services and rates.
• Maintain ongoing communication and provide timely updates to stakeholders regarding intake status.
• Ensure compliance with agency policies, regulatory requirements, payer guidelines, and documentation standards.
• Complete assigned Intake Workflow tasks within the EMR and identify optimization opportunities.
• Track and maintain accurate intake records, metrics, and reporting data to support goals.
