Job summary

Harris Computer is seeking a Remote Care Manager to oversee patient panels and coordinate care acrossPractice locations. This remote role requires an active nursing license (RN/LPN/BSN) and at least two years of clinical experience in clinic, Med/Surg, or home health.

The compensation is productivity-based, with tiered pay per patient encounters and performance bonuses. Strong communication, organization, and digital literacy are essential for success in a remote environment.

Qualifications

• Two years of clinical experience in a clinic setting, Med/Surg, Case Management, or home health.
• Current compact license to practice as RN/LVN/LPN in the applicant's state.
• Experience with Electronic Medical Records and Windows-based tools.
• Ability to learn new technology through self-guided training.
• Excellent verbal, written, and listening skills.

Responsibilities

• Abide by care plans and orders from the practice.
• Provide prevention and intervention for chronic conditions.
• Develop care plans for doctors and patients.
• Attend regular meetings and coordinate with the supervisor.
• Improve revenue by creating billable Care Management episodes.
