Job summary

Esrun Health is seeking a Remote Care Manager to manage a patient panel and perform care coordination for chronic conditions. The role requires an active compact RN/LPN/LVN license and at least two years of clinical experience in clinic settings, with EMR proficiency and HIPAA compliance.

Qualifications

• Current COMPACT license to practice as an RN/LV N/LPN.
• Minimum of two years of clinical experience in a clinic setting, Med/Surg, Case Management, and/or home health care.
• Hands-on experience with Electronic Medical Records and HIPAA compliance in home care.

Responsibilities

• Abide by the plan of care and orders of the practice.
• Provide prevention and intervention for chronic disease conditions.
• Develops positive interactions with patients on behalf of our practices.
• Improve revenue by creating billable Care Management episodes for chronic condition management.
• Develop detailed care plans for doctors and patients.
• Attend regularly scheduled meetings with supervisor and team.
