SelectQuote Insurance Services is hiring a Care Navigator to provide proactive chronic care management support from a remote location in the continental United States. You will conduct monthly patient calls, educate, and coordinate care across PCPs, specialists, and other providers using ourCare Navigator software.

The ideal candidate is a Certified Medical Assistant with 2+ years in healthcare, strong communication, and the ability to work independently in a fast-paced setting.

Qualifications

• Certified or Registered Medical Assistant required.
• 2+ years in healthcare or related field; 3–5 years preferred.
• Ability to work full-time (40 hours per week).

Responsibilities

• Provide proactive monthly patient calls and education.
• Review health status and escalate to physician per protocol.
• Set health-related goals and close care gaps.
• Create and update care plans in proprietary software.
• Maintain productivity and quality metrics.
