Job summary

Sharecare is seeking a Certified Health Coach to join a remote, full-time team. You will conduct education, support, and motivation to help members reach health goals using a structured outreach model. Training lasts six weeks with CST hours, and weekend rotation is required.

The role emphasizes coaching via chat and phone, documentation, and collaboration with nurses and vendors to improve client health outcomes. Requires NBHWC training and a health-related bachelor’s degree.

Qualifications

• Completed NBHWC certified training program required.
• Bachelor's degree in a health-related field requested.
• Active NBHWC board certification is a plus.
• Experience delivering coaching via chat is a plus.

Responsibilities

• Provide telephonic coaching calls including engagement and enrollment.
• Deliver behavior change coaching to reduce high-risk behavior.
• Reach out to enrolled members who lack a scheduled call.
• Coordinate care with nurses and external vendors.
• Support operational needs to meet customer requirements.
• Maintain daily and monthly meetings and track member progress.
