Job summary

Sharecare is seeking a Certified Health Coach to operate within a remote, full-time role. You will connect with program participants via telephonic coaching, educate and motivate clients, and document progress in our care platform.

Qualifications

• Bachelor's degree in a health-related field is required.
• NBHWC certified training is required for qualification.
• Active NBHWC board certification is a plus.

Responsibilities

• Engage in inbound and outbound coaching calls to enroll participants in the program.
• Provide individualized behavior change coaching to reduce high-risk behaviors.
• Coordinate delivery of services with a multi-disciplinary care team.
• Support scheduling and caseload management to meet program requirements.
• Attend daily and monthly meetings to share insights and progress.
• Document interactions and track participants' progress and efficacy of the program.
