Job summary

Sharecare is seeking a Certified Health Coach to perform telephonic coaching with program participants, providing education, support, and motivation to improve health behaviors. You will work within a structured, high-engagement outreach model and collaborate with nurses and vendors as part of a multi-disciplinary team.

Applicants should have NBHWC training with a health-related bachelor's degree and strong communication skills.

Qualifications

• Bachelor's degree in a health-related field is required.
• NBHWC certified training program is required for eligibility.
• Active NBHWC board certification is a plus and not mandatory.

Responsibilities

• Participate in telephonic coaching calls (inbound and outbound).
• Provide individualized behavior change coaching using motivational interviewing.
• Coordinate with multi-disciplinary care team to deliver services.
• Support operational aspects to meet customer requirements and manage caseloads.
• Attend daily/monthly meetings and present findings.
• Document interactions and track member progress.
