Job summary

CVS Health in Michigan seeks a Coordinator of Care to manage long‑term rehabilitation and psychosocial needs. You will coordinate all case management activities, engage with providers and care teams, and connect members to community resources to prevent readmissions.

You will develop individualized care plans, educate members about services, and collaborate with community partners to improve health outcomes.

Qualifications

Minimum 2 years experience in medical social work or case management.
Willingness to travel up to 25-50% in Wayne and Macomb Counties.
Reliable transportation and mileage reimbursement per policy.
Problem solving and digital literacy skills.
Knowledge of medical terminology.
Ability to handle sensitive information ethically and responsibly.
Ability to work with diverse cultural dynamics.

Responsibilities

Coordinate care for members with acute or chronic conditions to improve outcomes and reduce costs.
Assess members in Long-Term Care or case management programs.
Develop individualized case plans for at‑risk members.
Communicate with providers, PCPS, Medical Directors, and care staff.
Identify opportunities to ensure care aligns with benefit plans in home or community settings.
Provide information and education on social issues and available services.
Develop programs to reduce admissions and improve community health outcomes.
Coach junior colleagues in social work techniques.
Collaborate with community members and policymakers to address issues.
