Job summary

CVS Health is seeking a dedicated care coordinator to manage long-term rehabilitation and psychosocial needs, coordinating case management, home/community resources, and quality of life activities.

You will work with providers and care teams to optimize outcomes, develop individualized care plans, and advance community health initiatives. This is a full-time role with travel across Michigan's Wayne and Macomb Counties.

Qualifications

• Master's Degree in Social Work required by state law.
• LMSW licensure required or preferred.
• Travel up to 25%–50% in Wayne and Macomb Counties.

Responsibilities

• Coordinate care for members with long-term rehabilitation and psychosocial needs.
• Implement and coordinate case management, psychosocial, and QoL activities across care continuum.
• Communicate with providers and care staff about patient status and needs.
• Develop programs to reduce hospital admissions and improve health outcomes.
• Mentor junior social work staff and collaborate with community partners.
