Job summary

CVS Health seeks a Case Manager, Registered Nurse to join our Community Care team in a fully remote role across participating states. You will leverage best-in-class case management models to serve members, collaborate with providers and insurers, and drive outcomes in a member-centric care approach.

The role requires an active RN license, 3+ years of clinical experience, 2+ years in case management, and bilingual Spanish is preferred. CCM certification is encouraged.

Qualifications

• Active RN license in applicable states.
• 3+ years of clinical experience.
• 2+ years of case management, discharge planning, or home health coordination.
• Bilingual in Spanish preferred; CCM certification encouraged.

Responsibilities

• Acts as liaison with members, families, providers, and insurers.
• Coordinate case management across continuum of care.
• Assess member needs and develop plans to optimize outcomes.
• Document case work and communicate with stakeholders.
• Ensure compliance with regulations and carrier requirements.
