CVS Health is seeking a Case Manager RN for a fully remote, nationwide role. You will conduct clinical assessments, develop and monitor care plans, and assist with discharge planning, while collaborating with Medical Directors and care teams.

Expect regular 9:00–17:30 local time hours and occasional late shift once a month. The role requires an active RN license, ~3+ years acute care experience (Med-Surg/CCU), CCM certification, and a nursing degree.

Qualifications

• Active, unrestricted RN license in your state of residence.
• Approximately 3+ years of acute care RN experience (Med-Surg or CC preferred).
• Compact or multi-state RN license.
• Certified Case Manager (CCM).
• Telephonic case management experience.
• Associate Degree in Nursing (required).
• 100% remote—live in any state.
• No travel required.

Responsabilités

• Conduct comprehensive clinical assessments and identify care coordination needs.
• Develop, implement, and monitor individualized case management plans.
• Support inpatient review and discharge planning as appropriate.
• Identify and elevate quality‑of‑care concerns.
• Deliver exceptional customer experience while collaborating with care teams and Medical Directors.
