Qualifications

• Active and unrestricted Compact RN licensure in the state of residence.
• 3+ years of clinical practice with Medicare populations preferred.
• 2+ years of case management, discharge planning, or home health coordination.
• Proficient with Word, Excel, Outlook, PowerPoint and some proprietary applications.
• Ability to work in a fast-paced, high-volume environment with good time management.
• Strong communication and problem-solving skills.

Responsibilities

• Coordinate and facilitate comprehensive care; assess needs and develop care plans.
• Advocate for patients; ensure effective communication and resource use.
• Interact with members, providers, and insurers across the continuum of care.
• Document case work activities and monitor progress to promote positive outcomes.
• Collaborate with internal teams to optimize wellness and return to baseline.
