Job summary

CVS Health is seeking a Community Care Case Manager (RN) to assess, plan, facilitate, and advocate for individuals and families across the care continuum, using available resources to promote quality and cost-effective outcomes.

You will collaborate with members, providers, insurers, and employers; perform home visits or telephonic follow-up; develop care plans and document activities. This is a full-time remote role in the United States with standard hours and a competitive range.

Qualifications

• Active RN licensure in OH or state of residence
• 3+ years clinical experience (diabetes, CHF, CKD, post-acute) with Medicare members
• 2+ years case management or home health coordination experience
• Willingness to obtain multi-state RN licenses if needed
• National professional certification (CRC, CDMS, CRRN, COHN, CCM)

Responsibilities

• Liaises with members, families, providers, insurers and employers as appropriate
• Coordinates case management for catastrophic or chronic cases across the care continuum
• Interacts telephonically or in person; may visit homes, worksites or physicians' offices
• Assesses medical/vocational status and develops a care plan to optimize wellness
• Documents all case work activities and communicates with stakeholders
• Collaborates with a multidisciplinary team to maximize health outcomes
• May outreach to treating physicians or specialists as needed
• Provides education and prevention information for best outcomes
• Ensures compliance with laws, HIPAA and carrier requirements
• Monitors progress toward desired outcomes and adjusts plans accordingly
