Job summary

CVS Health is seeking a case manager to coordinate comprehensive care for members, build personalized care plans, and collaborate with providers and insurers nationwide. The role emphasizes advocacy, documentation, and proactive follow-up to improve outcomes and access to care.

You will assess needs, coordinate services, and monitor progress, supporting patients across settings while adhering to HIPAA and program guidelines.

Qualifications

• Active and unrestricted Compact RN licensure in state of residence.
• 3+ years of clinical experience (diabetes, CHF, CKD, etc.) with Medicare members.
• 2+ years of case management, discharge planning or home health coordination.
• Proficiency with Microsoft Word, Excel, Outlook, PowerPoint and related apps.
• Able to work in a fast-paced, high-volume environment with strong time management.

Responsibilities

• Coordinate case management activities across the continuum of care.
• Communicate with members, providers, insurers and other stakeholders.
• Prepare documentation of case work activities and plans.
• Assess needs and eligibility; develop care plans to promote wellness.
• Monitor progress and advocate for optimal health outcomes.
