Qualifikationen

• Bachelor's degree in social work or related field, or relevant experience.
• Experience in case management, housing services, benefits navigation, or social services.
• Ability to travel locally in San Diego County and work with diverse communities.

• Manage a caseload with ongoing client communication and support.
• Conduct comprehensive housing, financial, employment assessments.
• Develop and update individualized housing stabilization plans.
• Assist with documentation, eligibility, and access to financial assistance.
• Connect participants with rental assistance, benefits, and community resources.
• Coordinate with Housing Navigators to address complex housing challenges.
• Maintain progress notes and program outcomes in the system.
