This position works closely with participants to assess their needs, develop individualized stabilization plans, coordinate financial assistance and supportive services, and connect households with resources that promote long-term housing stability.

Case Managers collaborate with internal programs, community partners, and housing service providers to help participants overcome barriers, prevent displacement, and achieve greater housing and financial security. This position requires strong organizational skills, accurate documentation, and a commitment to providing compassionate, culturally responsive, and participant-centered services.

  • Assist participants in obtaining required documentation, completing eligibility assessments, and accessing available financial assistance and supportive services.
  • Connect participants with rental and utility assistance, employment opportunities, public benefits, financial education, legal services, and other community resources.
  • Coordinate with Housing Navigators and other program staff to address complex housing challenges, landlord‑related concerns, and barriers to maintaining stable housing.
  • Monitor participant progress, evaluate service outcomes, and adjust stabilization plans as needed.
  • Identify and promptly elevate urgent participant needs, including imminent housing loss, safety concerns, or other circumstances requiring immediate intervention.
Community Resources & Service Coordination
  • Collaborate with staff across City Heights CDC's housing, economic development, workforce development, and supportive service programs to connect participants with additional resources and opportunities.
  • Maintain knowledge of community assistance programs, tenant resources, affordable housing opportunities, and supportive services throughout San Diego County.
  • Develop and maintain productive relationships with community organizations, service providers, and referral partners to facilitate coordinated participant support.
  • Educate participants about available housing resources, tenant protections, and strategies for maintaining long‑term housing stability.
Documentation, Compliance & Program Performance
  • Maintain accurate, complete, and timely participant records, case notes, service plans, referrals, and outcomes in Salesforce and other required systems.
  • Ensure services and documentation comply with applicable funding and contract requirements, organizational policies, confidentiality standards, and established program procedures.
  • Meet established caseload, service delivery, documentation, and performance expectations while maintaining high quality services.
  • Participate in case conferences, team meetings, professional development activities, and quality assurance reviews to support continuous program improvement.
  • Maintain appropriate professional boundaries and provide trauma‑informed, culturally responsive, and equitable services to individuals and families from diverse backgrounds.
