Placement Partners is seeking a Case Manager to coordinate services, complete documentation, and communicate with clients, families, providers, and county teams. The role begins in office and transitions to fully remote after 90 days, with ongoing QA and paraprofessional support to assist with documentation and quality review.

Qualifications

• Bachelor’s or Master’s degree in Social Services, psychology, human services or related field.
• Reliable vehicle with up-to-date Driver’s License and Auto Insurance.
• Maintain an acceptable motor vehicle report.

Responsibilities

• Coordinate services for a caseload of 40–50 clients with QA and paraprofessional support.
• Develop and monitor person-centered support plans, goals and outcomes.
• Connect clients/families with services, providers and community resources.
• Advocate for clients and resolve service-related needs.
• Complete assessments, case notes and required documentation.
• Ensure services remain compliant with DHS regulations and company policies.
