Qualifications

Responsibilities

• Coordinate the individual's application, referrals, support planning and monitoring.
• Ensure compliance with HCBS Eligibility and Enrollment Processes.
• Assist with assessments and planning meetings for ISA/BIP.
• Facilitate development of service plans and budget context documents.
• Coordinate meetings and communicate status to the team.
