Job summary

CVS Health is seeking a Case Management Coordinator for a full-time telework role with up to 75% travel across specified counties. You will support members by scheduling appointments, guiding access to benefits, and coordinating care plans to improve outcomes.

The ideal candidate has 2+ years in behavioral health or social services and proficiency with MS Office. Schedules are Monday–Friday 8:00 am–5:00 pm CST; travel is reimbursed as per policy.

Qualifications

• 2 years+ experience in behavioral health or social services
• Willing and able to travel 50-75% in specified counties; reliable transportation.
• 1+ year(s) of experience with Microsoft Office Suite applications (MS Word, Excel, Outlook and PowerPoint)

Responsibilities

• Collaborate to inform the case management process and assist with appointment scheduling.
• Identify high risk factors and service needs; refer to clinical case management or crisis intervention as appropriate.
• Coordinate and monitor care plan activities and progress.
• Engage with team meetings and offer peer mentoring/training.
• Help members participate with their provider in healthcare decision-making.
