Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willing and able to travel 50-75% in Metro East, IL area.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members and determine needs/eligibility; propose care plan approach.
• Identify high risk factors and coordinate appropriate referrals.
• Coordinate and monitor care plan activities and progress.
• Consult with care management and medical directors to optimize outcomes.
• Identify and escalate quality of care issues through established channels.
• Engage members to promote healthy lifestyle choices and informed decisions.
• Document care processes in line with regulatory and company policies.
