Job summary

CVS Health is seeking a Case Management Coordinator for a full-time telework role requiring 50-75% travel in Lake, McHenry and Northern Cook Counties, Illinois. You will collaborate with care teams to evaluate member needs, assist with benefits access and education, and guide care management using available resources.

Standard hours are Monday through Friday, 8:00am–5:00pm CST. You will coordinate care plans, monitor progress, and escalate issues to ensure quality of care and optimal outcomes

Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willingness to travel 50-75% in Lake, McHenry, and Northern Cook Counties Illinois.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluate members' needs/eligibility and recommend case resolution.
• Identify high risk factors; refer to clinical case management or crisis intervention as appropriate.
• Coordinate and implement care plan activities; monitor progress.
• Collaborate with Medical Directors and health programs to improve care outcomes.
• Identify and escalate quality of care issues through established channels.
• Use negotiation and motivational interviewing to secure services and engage members.
• Provide coaching and support to empower informed health decisions.
• Engage in team meetings and peer mentoring.
• Assist members in active, informed participation with providers.
