Devoted Health is seeking a Transitions of Care Coordinator to support TOC Clinical Operations with inpatient outreach and post-discharge follow-up. You will help schedule follow-ups, coordinate documentation and orders, and ensure patients are prepared for telehealth visits.

Two-plus years in patient-facing care coordination and telemedicine experience are preferred. This role emphasizes collaboration with doctors, nurses, and caregivers and offers a dynamic, growth-focused environment.

Competenze

• 2+ years of patient facing and care coordination experience in healthcare.
• Experience working in a telemedicine environment is preferred.
• MA certification or active MA credentials are desirable.

Mansioni

• Reach out to patients inpatient to introduce TOC program and outline follow-up.
• Schedule post-discharge follow ups with PCPs and specialists.
• Obtain clinical documentation from hospitals and provider offices.
• Submit Home Care and DME orders; follow up with vendors.
• Coordinate lab tests and imaging and track results for clinician review.
