Devoted is seeking a TOC Supervisor to lead a team of TOC Coordinators, ensuring patients transition safely from hospital to home or post-acute settings. You will advocate for patients, coordinate services (appointments, transportation, supplies, DME), and participate in rounds and leadership meetings.

Your role blends clinical touch with operations, dedicating about 30% of time to direct TOC patient care while mentoring staff, optimizing workflows, and driving performance across the TOC program.

Qualifications

• 2 years direct lead or supervisory experience in a virtual setting.
• Healthcare administrative/office experience 2+ years.
• MA Certification (active or expired) required; verification may be requested.
• Fully bilingual English/Spanish preferred but not required.

Responsibilities

• Lead a team of TOC Coordinators and support clinical team.
• Provide TOC patient care 30% of the time; coordinate resources and services.
• Recruiting/hiring support for new TOC Coordinators.
• Develop new workflows and job aids; coach and manage performance.
• Attend/host regular meetings with team patients and leadership.
• Monitor KPIs; ensure attendance and participation in meetings.
