Job summary

Qualifications

• High School diploma or GED required.
• Two years of clinical setting experience required.
• College degree preferred.
• Medical Assistant certification and/or related experience preferred.

Responsibilities

• Create and distribute daily patient activity reports for provider offices.
• Review, authorize, and assign referral requests.
• Monitor level of care determinations and track authorizations.
• Conduct telephonic outreach to ensure smooth transitions of care.
• Schedule appointments and coordinate transportation as needed.
• Maintain collaborative relationships with care teams and PHO staff.
