Job summary

Optima Medical is seeking an in-office Care Coordinator to join our CCM program. You will work directly with patients living with chronic conditions, helping them stay on track with care plans and health goals through regular outreach and follow-up using EMR systems.

The role emphasizes coordination, education, and collaboration with physicians, nurses, and social services to support patients’ overall well-being while leveraging clinical knowledge daily.

Qualifications

• 1+ year of clinical work experience as a Medical Assistant
• 1 year of EMR experience; eClinicalWorks preferred
• High level of professionalism
• Excellent written and verbal communication skills
• Strong customer service skills
• Medical Assistant Course Completion Diploma or CMA designation required
• Bilingual English/Spanish is a plus
• Must have HS diploma or GED equivalent

Responsibilities

• Develop personalized care plans with patients, caregivers, and providers.
• Coordinate and facilitate communication among physicians, nurses, and support staff.
• Monitor adherence to treatment plans and medications.
• Educate patients and families on disease management and preventive care.
• Document encounters using EHR and track interventions.
• Collaborate with community resources to address social determinants of health.
• Conduct regular follow-ups to optimize health outcomes for chronic conditions.
