Qualifications

• Associate's degree in Healthcare or related field.
• Minimum two years of experience in a directly related healthcare role.
• Proficiency with Microsoft Office applications and web-based technologies.
• Comfortable learning proprietary platforms.
• Basic Excel proficiency (pivot tables, formulas).
• Flexible schedule within assigned market hours.
• Experience coordinating care for members.
• Familiarity with healthcare laws and health information protection.

Responsibilities

• Conduct outbound calls to members to assess health needs and determine care coordination eligibility.
• Assist patients with barriers to care, scheduling appointments, and accessing community resources.
• Collaborate with patients, providers, referral coordinators, and staff; document activities accurately.
• Communicate available care coordination services to patients and caregivers.
• Screen for physical and behavioral health needs and refer to appropriate professionals.
• Provide data to providers, patients and families to support informed decisions.
