Job summary

Equality Health seeks a Care Specialist (Texas) to conduct outreach, assess health needs, and coordinate care for diverse patient populations. This remote, full-time role operates in the Texas market with local hours and alignment to market time zones.

Qualifications

Associate’s degree in Healthcare or related field or equivalent experience.
Minimum two years of experience in a healthcare-related position.
Proficiency with Microsoft Office and web-based technologies.
Strong outbound calling experience and ability to work independently.

Responsibilities

Conduct outbound calls to members to assess health needs and eligibility for care coordination programs.
Assist patients with barriers to care, including scheduling appointments and accessing resources.
Document all activities and collaborate with network providers and care staff; communicate available services to patients and caregivers.
Screen for health needs and refer to appropriate professionals for timely intervention.
Participate in ongoing education to maintain skills and competency.
