Job summary

Equality Health is seeking a Care Specialist to provide supportive care to a diverse patient population, identify barriers to self-care, and assist with navigation of the healthcare system. You will work with Community Health Workers, Chaplains, and our Clinical Operations team, upholding confidentiality and professionalism.

Qualifications

• Associate degree in Healthcare or related field; minimum two years of experience
• Proficient with Microsoft Office applications and web-based technologies
• Comfortable learning proprietary platforms
• Basic Excel proficiency (pivot tables, formulas)
• Flexible schedule within assigned market hours
• Previous experience coordinating care for members
• Familiarity with federal and state health data laws
• BLS, CMA, CMA, EMT certifications
• Excellent problem-solving and independent work
• Bilingual preferred; call center experience acceptable

Responsibilities

• Conduct outbound calls to assess health needs and determine participation in care programs
• Assist patients with barriers to care including scheduling appointments and accessing resources
• Collaborate with patients, providers, referral coordinators, and staff; document activities
• Communicate care coordination services to patients and caregivers
• Screen for physical and behavioral health needs and refer appropriately
• Provide data to providers, patients and families
