• Minimum two years in a healthcare-related role.

Responsibilities

• Conduct outbound calls to members to assess health needs and participation eligibility.
• Assist patients with barriers to care including scheduling and accessing resources.
• Collaborate with patients, providers, and staff; document all activities clearly.
• Explain available care coordination services to patients and caregivers.
• Screen for physical and behavioral health needs to refer to clinicians.
