Equality Health is seeking a Care Specialist for remote work, primarily serving a designated market with hours aligned to local time. Responsibilities include outreach calls, coordinating care, and helping patients access services.

Ideal candidates have healthcare experience, strong communication skills, and proficiency with Microsoft Office. Familiarity with healthcare regulations and bilingual abilities are preferred.

Formación

• Associate’s degree in Healthcare or a related field is required plus minimum two years in a related healthcare role.
• Proficiency with Microsoft Office applications and web-based technologies.
• Comfortable learning proprietary platforms and basic Excel proficiency (pivot tables, formulas).
• Flexible schedule within assigned market hours and the ability to coordinate care for members.

Responsabilidades

• Conduct outbound calls to members to perform health assessments and determine eligibility for care coordination programs.
• Assist patients with barriers to care, including scheduling appointments and locating community resources.
• Collaborate with patients, providers, referral coordinators, and staff; document all activities.
• Communicate available care coordination services to patients and caregivers.
• Screen for physical and behavioral health needs to refer to appropriate healthcare professionals.
• Provide meaningful data to providers, patients, and families.
