Job summary

Spectraforce Technologies, Inc. is seeking a Resource Specialist to support remote field-based care management.

You will assess member needs, develop or contribute to care plans with the care team, and coordinate referrals for services such as employment, housing, and community supports, aligning with member goals and preferences. This position is remote/field-based, with a caseload of up to 100 members, and requires 8 am-5 pm, Monday–Friday availability; weekend work may be needed during

Qualifications

Bachelor's degree required with 2–4 years of related experience.
Candidate should have health plan or child welfare experience and be able to connect families to local resources.

Responsibilities

Assess member needs and develop or contribute to care plans with the care team.
Assist with connecting members to local resources for services (employment, housing, independent living, justice, foster care).
Act as a resource for members seeking local resource services and determine eligibility based on assessments.
Monitor delivery of care/services and follow up to verify timely access.
Collaborate with providers, community organizations, and governmental entities to support access to services.
Develop education and training programs for care management staff and providers to improve resource identification.
Draft materials and assist in developing local resource services, policies, and procedures.
Document all resource services information and keep it up to date.
May conduct outreach to locate hard-to-reach individuals and families.
