Qualifications

• Bachelor's degree required with 2–4 years of related experience.
• Pediatric case management experience preferred.
• Experience connecting families to local resources for employment, housing, independent living, justice, foster care, etc.

Responsabilités

• Provides resource support to members and connects members to local resources based on assessments and plans.
• Identifies best local resources for members based on industry best practices.
• Performs care management tasks including health care needs assessment and referrals to plan providers.
• Documents outreach, assessments, and care plans in electronic health records.
