The Care Manager will handle an active caseload (~73 Behavioral Health Complex members), coordinate with clinicians and providers, and maintain EMR documentation, care plans, and medication updates to close care gaps. Flexibility and collaboration with community partners are essential.

Qualifications

• Requires Master’s in Behavioral Health or Social Work or Nursing degree from an accredited school.
• Active Florida Behavioral Health License (LMHC, LCSW, LMFT) is required.
• 2–4 years of related experience preferred.

Responsibilities

• Maintain an active caseload of about 73 Behavioral Health Complex members.
• Outreach to members, primary care physicians, and outpatient providers biweekly.
• Initiate assessments using the internal EMR system and create personalized care plans.
• Update medications, diagnoses, and allergies in the EMR monthly.
• Review care gaps and provide education/resources to close gaps.
• Connect members to community providers and resources.
• Attend community staffing and treatment teams; collaborate with agencies.
• Staff complex members with internal medical directors and clinical staff as needed.
