Responsibilities

• Assess member needs and develop or contribute to care/service plans with the care team.
• Connect members to local resources and assist with documentation and applications as needed.
• Coordinate with providers and community resources to ensure timely access to care/services.
• Monitor delivery of care/services and follow up with members and caregivers to verify transitions.
