Spectraforce Technologies is seeking an LTSS Service Care Manager to support NC Region 2 members with complex medical needs. This role focuses on developing and coordinating holistic care plans and connecting members and families with available services.

The position combines remote work with required face-to-face home visits, collaboration with care teams, and potential for travel within the region. Bachelor’s degree and licensure (RN or LCSW) are required or preferred.

Competenze

• Bachelor’s degree and 2–4 years of related experience including LTSS/HCBS coordination or care management.
• RN or LCSW license is required; RN or LCSW is preferred.
• Must have two years LTSS/HCBS coordination or care management experience; experience in social work, geriatrics, gerontology, pediatrics, or human services.
• Nice to have: prior managed care experience.
• Disqualifiers: lack of required experience, non-licensed, unsuccessful background check.

• Evaluate the needs of the member, resources, and plan for the best outcome.
• Assist with developing long-term care plans and identify providers and community resources.
• Coordinate between member/family/caregivers and care provider team to ensure services are accessible.
• Provide resource support for employment, housing, participant direction, independent living, justice, foster care, etc.
• Monitor care plans and member status, provide recommendations, and adjust plans as needed.
• Interact with LTSS care providers and partners to meet member needs.
• Collect, document, and maintain member information and care management activities for regulatory compliance.
• Perform home and/or site visits to assess needs and collaborate with providers.
• Educate members and caregivers on procedures, provider instructions, service options, referrals, and benefits.
• Provide feedback to leadership on opportunities to improve quality and cost-effective service delivery.
• Perform other duties as assigned.
