Job summary

Responsibilities

• Provide holistic, evidence-based care including assessment and treatment.
• Evaluate patients 18 months and older across primary/acute/chronic care.
• Counsel on contraception, STI prevention and safer sex practices.
• Document care in the EHR per clinic policies.
• Coordinate care with internal and external colleagues.
