Job summary

CVS Health seeks a Minute Clinic Virtual Care Nurse Practitioner (Provider) for a 6-month PRN contract. Remote role with 20+ hours per week, including some weekends; active US licensure required and possible licensure expansion.

You will deliver holistic, evidence-based care via a remote platform, assess, diagnose, treat, educate and coordinate care for patients aged 18 months and up, collaborating with a multidisciplinary team.

Qualifications

• Active, unrestricted nursing licensure in any US state.
• Minimum of two years of medically-relevant experience.
• Master’s Degree as Family Nurse Practitioner with current certification.
• Strong verbal, written, and electronic communication skills.
• Ability to collaborate with health-care professionals to provide quality care.

Responsibilities

• Provide holistic, evidenced-based care including assessment, diagnosis, treatment, and disposition planning.
• Evaluate primary care, acute, chronically ill, and transitional care patients aged 18 months and older.
• Counsel on contraceptive care and pregnancy prevention and manage medications.
• Educate patients on health maintenance and respond to inquiries.
• Document all patient care in an EHR per company policies.
• Coordinate care with internal and external colleagues to ensure high-quality care.
