Job summary

CVS Health is seeking a Minute Clinic Virtual Care Nurse Practitioner (Provider) to deliver holistic, evidenced-based care remotely for patients aged 18 months and older. You will work with a dedicated team, providing assessment, diagnosis, treatment, counseling, and disposition planning via a remote technology platform.

The role is a full-time evening position (32 hours/week) with every other weekend, shift 2:30 p.m.–11:00 p.m.

Qualifications

• Current license in one of: D.C., DE, FL, KS, KY, MA, MD, ME, NH, OH, RI, VT
• Minimum of two years of medically-relevant experience
• Effective verbal, written, and electronic communication skills
• Outstanding organizational skills and ability to multi-task
• Initiative, problem solving ability, adaptability and flexibility
• Ability to work without direct supervision and practice autonomously
• Proficient with information management and technology
• Capacity to collaborate with professional colleagues to provide quality care
• Spanish language proficiency may be required in some markets

Responsibilities

• Provide holistic, evidenced-based care including assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients 18 months and older
• Evaluate primary care, acute, chronically ill, and transitional care patients; provide education and counseling
• Provide patient counseling including contraception care counseling and medication management
• Educate patients on health maintenance and respond to inquiries
• Document all patient care in an EHR per MinuteClinic policies
• Coordinate care with internal and external colleagues across the patient-centered medical home
• Work with care team including Providers, physicians, paraprofessionals, pharmacists
• Work independently, prioritize and solve problems, advocate for patients and practice
