Community Healthcare Network, Inc. is seeking a Patient Navigator for its Health Justice Mobile Program. The role supports patient engagement, registration, insurance verification, care coordination, and linkage to services for clients served by the mobile unit.

Under supervision of the Program Manager, you will be a key contact for patients accessing services. This position is funded through a local District Attorney’s Office and employment contingent on funds.

Qualifications

• High school diploma or equivalent.
• Diploma or coursework in health, human services, public health, social work, psychology, or related field preferred.
• Experience in health care, mental health, community health, or human services setting required.
• Experience using an EMR system and registering patients preferred.

Responsabilités

• Greet, engage, and support patients accessing services through Health Justice Mobile Program.
• Complete patient registration, intake, demographic updates, and required documentation in the EMR.
• Conduct outreach and follow-up calls to encourage appointment attendance and linkage to services.
• Verify health insurance eligibility and connect patients to CHN staff for insurance questions.
• Schedule appointments and coordinate follow-up visits with CHN providers.
• Assist with patient flow on the mobile unit to support timely service delivery.
• Provide information about health, mental health, sexual health, and supportive services clearly and respectfully.
• Maintain accurate records and protect patient confidentiality in accordance with policies.
