Mountain Area Health Education Center is seeking a Care Navigator to join its Population Health Administration embedded in primary care teams. You will coordinate care, engage patients and families, and connect them with resources to meet personal health goals.

Responsibilities include documenting plans in the EHR, referring to community resources, supporting care coordination, and contributing to quality improvement through team-based workflows.

Qualifikationen

• Associate’s degree or equivalent in a health-related field and/or related licensure.
• Undergraduate coursework in social sciences or related certification preferred.
• Bachelor’s degree in social work or related field preferred; three years healthcare experience may substitute.

• Engage with patients and families to obtain histories and document plans.
• Build strong relationships to support patient engagement during visits and virtual sessions.
• Use motivational interviewing to gather information and support self-management goals.
• Identify barriers to care and coordinate with providers to suggest alternatives.
• Document activities and care plans in the EHR and care management platform.
• Refer patients to educational resources, housing options, government programs, and community services.
• Collaborate with leadership to define workflows for screening, outreach, and care coordination.
• Utilize team-based care to ensure depression and social determinants of health screening and follow-up.
