Millennium Physician Group is seeking a Care Navigator II to support patients and families with navigation, coordination of care, and follow-up across settings. The role works under the RN Care Manager, handling routine and moderately complex cases in a primarily remote outpatient environment.

The position requires a Florida LPN license or CMA/RMA certification, with at least 3 years of clinical healthcare experience and proficiency with EHR systems.

Qualifikationen

• Current Florida Licensed Practical Nurse (LPN) license or Certified Medical Assistant (CMA/RMA) certification from an accredited program (Preferred).
• Minimum of three (3) years of clinical healthcare experience in a physician practice or outpatient setting is preferred.
• Experience with electronic health records (EHR) is required.

Aufgaben

• Assist the RN Care Manager in implementing and monitoring individualized patient care plans.
• Perform monthly patient chart reviews to identify care gaps and preventive needs.
• Monitor and track follow-up appointments, referrals, testing, and care plan goals.
• Coordinate services among primary care providers, specialists, hospitals, SNFs, home health agencies, and community resources.
• Conduct telephonic outreach to patients and caregivers as directed by the RN Care Manager.
