Job summary

Lehigh Valley Health Network (LVHN) seeks an experienced nurse to implement care plans tailored to patients’ physical, mental, and social needs. You will coordinate transitions, support disease management, and educate patients and families across the continuum of care.

The role involves working with a multidisciplinary team, leveraging EMR tools, and navigating post-acute care. This is a remote position in Pennsylvania, with day shifts and salary competitive for a professional RN case manager.

Qualifications

• Bachelor’s Degree in Nursing, or equivalent experience
• 5 years RN experience and 2 years in case management or similar
• 2 years EMR experience
• Ability to coordinate care plans
• Ability to interact with diverse populations
• Knowledge of computer applications and analytical tools

Responsibilities

• Identify patients not meeting clinical goals and arrange follow-up per protocol.
• Review and update medication lists and document allergies in the EHR.
• Use registries and data to assist care decisions and support quality improvements.
• Identify variances in standards of care and system problems.
• Establish and maintain relationships with hospitals and post-acute facilities.
• Provide care navigation services to specialty populations and programs.
• Collaborate with patient/family to set mutual goals.
• Address barriers in healthcare access and coordinate services.
