Vivo Care in the United States is hiring a Care Navigator to build trust with patients, triage vital signs, and promote proactive health changes via remote patient monitoring and care coordination. You’ll collaborate with the patient’s care team to outline patient-centered goals and develop action plans over the phone.

The role requires 1–3 years of clinical experience, an unrestricted RN/LPN/LVN license in a compact state and New York, and comfort with telehealth platforms.

Formación

• Unrestricted RN/LPN/LVN license in a compact state and New York.
• 1–3 years of clinical experience, patient management, or disease management.
• Experience with Medicare patients and telephonic care management.

Responsabilidades

• Manage physiological markers like blood pressure/weight/glucose with clinical appropriateness.
• Lead monthly wellness calls and collaborative health goal setting.
• Provide preventive health education and coaching; escalate urgent care needs as required.
