Zusammenfassung

Millennium Physician Group, part of Mosaic Health, is seeking a Care Navigator II to support patients and families through navigation of appointments, care team communication, and community resources in a remote outpatient setting.

Under the direction of the RN Care Manager, you will assist with care coordination, transitions of care, chronic disease management, patient outreach, and documentation to improve patient outcomes and reduce avoidable utilization.

Qualifikationen

• Current LPN license in Florida or CMA/RMA certification preferred.
• Minimum 3 years of clinical healthcare experience in outpatient or care management settings.
• Experience with chronic disease management and care coordination is preferred.
• Knowledge of EHR systems and care transition processes is advantageous.

Aufgaben

• Assist RN Care Manager in implementing individualized patient care plans.
• Conduct monthly chart reviews to identify gaps and intervention opportunities.
• Coordinate services across providers, hospitals, and community resources.
• Perform patient outreach to address barriers to care and encourage engagement.
• Maintain accurate documentation in the EHR and track care management activities.
