Job summary

Mosaic Health, LLC is seeking a Care Coordinator II (non-RN) to support outpatient care management in Jacksonville, FL. You will collaborate with the RN Care Manager and care team to coordinate care plans, outreach, and transitions across settings.

You will educate patients, perform chart reviews, arrange referrals, and document in the EHR while addressing access barriers and coordinating community resources.

Qualifications

• Support care coordination and transition of care under supervision.

Responsibilities

• Assist RN Care Manager in implementing and monitoring individualized patient care plans.
• Perform monthly patient chart reviews and identify care gaps.
• Monitor follow-up appointments, referrals, and care plan goals.
• Coordinate services among PCPs, specialists, hospitals, and community organizations.
• Conduct telephonic outreach to patients and caregivers as directed.
• Assess barriers to care including transportation, medication access, and health literacy.
• Encourage participation in preventive services and chronic disease programs.
• Document accurately in the EHR and upload records from providers.
• Advocate for patient needs and coordinate appropriate services.
