UnitedHealth Group is seeking a Telephonic Care Navigator in Missouri. This full-time, remote role requires handling member outreach, educating on care gaps, and coordinating provider appointments while ensuring privacy and quality service.

You will train for four weeks and then work standard 9:00am–5:30pm local hours, with occasional overtime. Benefits and a supportive team environment are highlighted.

Formación

• 1+ years of call center and/or telephonic customer service experience.
• 1+ years of professional office experience using phone and computer as primary tools.
• 1+ years of healthcare/insurance experience or social work/community outreach.
• Proficient with Word, Excel, and Outlook.
• Ability to work from home with high-speed internet.
• Full-time availability Monday–Friday, during local business hours.

Responsabilidades

• Performs telephonic outreach to DSNP/CSNP members via multiple channels.
• Educates members on gaps in care and helps schedule provider visits.
• Assists with social determinants of health and links to resources.
• Ensures access to PCP and adheres to program guidelines.
• Delivers excellent customer service to members and providers.
• Maintains schedule adherence and privacy per HIPAA.
