Responsabilidades

• Provide member outreach by phone or home visits to discuss care/service plans and next steps.
• Coordinate care activities with providers, partners, and family/caregivers.
• Respond to member and provider inquiries regarding plans and services.
• Document and maintain member records per state/regulatory requirements.
• Refer to Care Manager or community resources as needed.
• Educate members on benefits and available resources.
• Adhere to policies and standards and perform related duties.
