UnitedHealth Group in Greensboro, NC is seeking a Telephonic Care Navigator (1063468) to support DSNP/CSNP members from a remote NC location. You will conduct telephonic outreach, educate on gaps in care, link to resources, schedule provider visits, and ensure access to PCP while meeting performance metrics.

Training lasts four weeks (9:00am–5:30pm Eastern) with two weeks in classroom and two weeks on the job.

Qualificações

• High School Diploma / GED OR equivalent work experience
• Must be 18 years of age OR older
• 1+ years of call center and / OR telephonic customer service experience
• 1+ years of professional office experience using telephone and computer
• 1+ years of Healthcare / insurance experience and / OR Social work / community outreach / advocacy experience
• 1+ years of experience with analyzing and solving customer problems; Word/Excel/Outlook proficiency
• Ability to work from home (high-speed internet)
• Ability to attend 100% of 4 weeks training; full-time Mon-Fri 9:00am–5:30pm local time; possible overtime

Responsabilidades

• Completes telephonic outreach to DSNP/CSNP members using multiple outreach modes
• Educates member on gaps in care and assists with closure of gaps, including scheduling provider appointments
• Assists members with social determinants of health and links to community resources
• Ensures member has access to PCP
• Outreaches members per program guidelines
• Meets metrics for quality and performance
• Provides excellent customer service to members and providers
• Maintains schedule adherence and good attendance
• Maintains confidential health information per HIPAA and regulations
